At the seminar, you will receive:
1. A Medical Release Form for us to obtain your medical
records from your Primary Care
Physician for the care you have received over the past
5 years. You should also have
information from any specialists you have seen during
that time.
If there is a charge for these records, you can have them sent directly to us,
usually at no charge. These records will help us document any illnesses
related to obesity. This is very important, as this information will help us
obtain insurance approvals.

2. Answer our Patient Questionnaire that will assist us in writing the letter to
your insurance company asking for surgical approval.

Once we have the above information and a copy of your insurance card, we will
contact your insurance company to determine if the Lap-Band® Procedure is
covered. We will notify you of their decision as soon as we have confirmation.
However, this may take weeks. Although we cannot guarantee your insurance will
cover the costs of the procedure, an accurate assessment of your medical history
gives us the best chance. If your insurance coverage is denied, you may decide to
pay cash, or evaluate from several financing options that are available.
It is possible that you may need to see a pre-op physician, such as your family
doctor, to make sure you are medically cleared for the surgery.
